The Clinical Care Coordinator supports patient care management efforts across Ruby Valley Medical Center’s two rural health clinics. This position plays a critical role in advancing goals aligned with chronic disease management, and preventative care outreach. The Coordinator will serve as a liaison between patients, providers, and the broader care team—facilitating care transitions, promoting quality improvement, and ensuring patients remain engaged and informed in their healthcare journeys.
